Abstract

Purpose: This study was performed to evaluate the peri-implant bone resorption with delayed and immediate loading of fixed mandibular complete denture using All-On-Four Implant concepts. Subjects and methods: Ten completely edentulous patients were chosen with ages ranging from 55-70 years. Each patient received maxillary complete denture and a screw retained fixed mandibular complete denture using All- On-Four implants, patients were equally divided according to the loading protocol into two groups: Group (1) implants were loaded by screw retained fixed mandibular complete denture three months after implants placement (delayed loading), group (2) implants were loaded by screw retained fixed mandibular complete denture one week after implants placement (immediate loading). Cone beam computed tomography was used to evaluate the peri-implant bone resorption at the time of denture insertion, 3and 6 months. Results: The results revealed that delayed loading group showed statistically significant lower amount of bone loss than immediate loading through out the study period. The results also showed that, at complete denture insertion there was no statistically significant difference between lingual marginal bone height measurements in the both groups. After three and six months, bone height measurements showed statistically significant increase in delayed loading group than immediate loading. Conclusion: Delayed loading showed better results than immediate loading, as favourable radiographic outcomes were recorded regarding crestal bone height changes with All-On-Four Implant system.
